Required S1918 Training

All copyright and other intellectual property rights subsisting in the S1918 video, including without limitation all text, images, graphics and code contained in the course, and in its look and feel ("Contents") are owned by Group. Except where otherwise specified, you may view, copy and print the Contents only for your own use for this Course and related to the performance of duties stemming from this Course. The Group reserves the right, but has no obligation, to change the Contents at any time.

Other than as specified above, neither the video nor any of its Contents may be modified or copied in whole or part in any form, including by framing, incorporation into other websites or other publication, or be used to create any derivative work. No links to the Course website may be included in any other website without the Group's prior written permission. Other than as specified above, nothing in this Course should be construed as granting any right or license. None of the Contents may be changed, nor may any copyright or author attribution notice appearing on any of the Contents be altered or removed, without the Group's prior permission.

Training Video

Each institution is required to have at least one person complete the S1918 FIL training to obtain CTSU approval for patient registration to S1918. 

The following S1918 FIL training should be completed by the site staff responsible for administration of the FIL Tool. After viewing the training presentation, please complete the short form at the bottom of this page to obtain credit for completion of the training. A training confirmation will then be displayed. This page serves as acknowledgement from SWOG that your site has completed the required training. Print the confirmation page and upload it via the CTSU Regulatory Submission Portal to complete the site training prerequisite.


Verification

For your site to obtain credit for completing the FIL training, complete the short verification form below. All fields except Comments are required.

Important: Please ensure the correct CTEP Site Code is indicated in the form below. After clicking on “Submit” below, the confirmation page, with the appropriate CTEP Site Code, must be submitted via the CTSU Regulatory Submission Portal to complete the site prerequisite to access the OPEN system for patient registration on S1918.  See Section 13 of the protocol for additional information.
