Training Webinar
Each institution is required to have at least one person complete the S1820 training webinar, below. If the designated trained staff leaves the institution, sites will need to provide access and completion of training for all new personnel participating in this protocol prior to any new enrollment of patients.
The webinar will have a post-test. You must answer all questions and get a 100% to receive credit. The post-test can be taken as many times as needed to pass. After viewing the webinar and passing the post-test, please complete the AIMS-RC Run-In Welcome Packet request form (http://j.mp/2EKjuEk) and the verification short form at the bottom of this page. A confirmation page will then be displayed which will serve as acknowledgement from SWOG that your site has completed the required training. Please print the confirmation page and file in your training file. To receive credit for this Protocol Specific Requirement (PSR), please submit a copy of the 100% Post Test confirmation page via the CTSU Regulatory Portal.

AIMS-RC Run-In Welcome Packet Request Form
After completing and passing the post-test, complete the following link: http://j.mp/2EKjuEk. Submission of this form will serve as verification of completion to the AIMS-RC team. The team will be prompted to send AIMS-RC Run-In Welcome Packets to your site. Please note: Only one staff member from each clinic site needs to order the AIMS-RC Run-In 'Welcome' Packets. If other staff members from your site will complete, or have already completed, S1820 webinar training, please coordinate so that only one person submits the request for these materials.
