CTEP Site (Institution) Codes

Each clinical site that participates in CTEP or DCP-sponsored clinical trials must be uniquely identified with a CTEP site code. This includes:

  • Institutions meeting the federal regulations and guidelines of engaged in research
  • Institutions holding a federal grant
  • Unique geographic location that requires drug shipment even if that site is legally part of another entity and conducts research under the parent entity’s FWA
  • Institutions relying on the NCI Central Institution Review Board  (CIRB)
  • Direct receipt of investigational agent shipments from CTEP’s Pharmaceutical Management Branch (PMB)
  • Enrollment and treatment of patients/research participants
  • Institution’s whose employees, representatives, and/or agents are authorized to obtain informed consent from patients
  • Direct receipt of federal funds
  • Directly responsible for collection/submission of data to the study sponsor or their designee

To apply for a CTEP Site Code, you will need to submit the following information on memo/letterhead to the Enterprise Core Unit (ECU) helpdesk at ecuhelpdesk@mail.nih.gov.

  • Complete name(s) of institution
  • Complete address
  • Website (if available) for verification
  • FWA number (required)
  • Public Research Contact information (email and/or phone) (for publication of site trial participation on public websites such as clinicaltrials.gov).  For the Public Research Contact information – this is usually a phone or email that patients would use to receive information on clinical trials at the institution.

Institution names are created to represent the location where patients are seen. The ECU will combine names such as adding a cancer center to a medical center’s name to show a relationship between them. Each location/site may be listed by the name shown on their website or by the NCI-Designated Cancer Centers list/FWA number and/or what the patient sees when visiting the location. ECU will use identifiers such as part of the street or city name in cases with institutions that have multiple locations.

To update a CTEP Site Code, the ECU helpdesk (ecuhelpdesk@mail.nih.gov) will need the following information on memo/letterhead:

  • Complete name(s) of institution – Old and New
  • Complete address(es) – Old and New
  • Website (if available) for verification
  • FWA number (required)
  • Public Research Contact information (email and/or phone) (for publication of site  trial participation on public websites such as clinicaltrials.gov).  For the Public Research Contact information – this is usually a phone or email that patients would use to receive information on clinical trials at the institution.
