Radiation Therapy Facility Approval
SWOG requires radiation therapy facility approval before patients can be registered to Group trials containing radiotherapy as an aspect of treatment. Approval of a facility requires the completion of a Radiation Therapy Facility Approval form. This form must be completed by the radiotherapy department and then forwarded to Dr. Paul Okunieff (Radiation Therapy Committee Chairman) at University of Shands Cancer Center. His address is provided on page 1 of the facility approval form. He will review this form and contact you directly as to provisional approval or request for additional information. Any suggestions for improvement must be addressed before approval is awarded. If more than one radiotherapy facility will be used to treat patients on SWOG protocols, a Radiation Therapy Facility Approval form must be completed for each facility.
